While it can affect anyone, gout is more common in men and individuals with specific risk factors such as obesity, excessive alcohol consumption, and a diet high in purines. What Is Gout?

Gout is a form of inflammatory arthritis that occurs when high levels of uric acid in the blood cause the formation of sharp urate crystals in the joints. These crystals lead to inflammation and intense pain during a gout flare-up. The most commonly affected joint is the big toe, but gout can also affect other joints such as the knees, ankles, elbows, wrists, and fingers.


Understanding Uric Acid and Gout

Uric acid is a waste product that forms when your body breaks down purines, substances found in certain foods and drinks like red meat, shellfish, and alcohol (especially beer). Under normal circumstances, uric acid dissolves in the blood and is excreted by the kidneys in the urine. However, when the body produces too much uric acid or the kidneys do not remove enough, the excess uric acid can accumulate and form sharp crystals in the joints.

Common Symptoms of Gout

Gout is known for its sudden and intense flare-ups, often occurring at night. The symptoms usually affect one joint at a time, with the big toe being the most common. However, gout can affect any joint, including the ankles, knees, wrists, elbows, and fingers. Here are the common signs and symptoms:

 

Sever Joint Pain

The hallmark of gout is sudden, intense joint pain that usually starts at night. The pain often centers around the big toe but can also affect other joints like the ankles, knees, wrists, and fingers.

 

Swelling and Inflammation

The affected joint will become swollen, red, and warm to the touch. This inflammation can cause significant discomfort and tenderness, even with the slightest touch.

 

Prolonged Discomfort

After the most severe pain subsides, some joint discomfort may linger for a few days to a few weeks. Subsequent flare-ups can last longer and may affect multiple joints.

 

Limited Range of Motion

During a gout flare, the affected joint may become stiff, making it difficult to move the joint normally. In chronic gout, repeated attacks can lead to permanent joint damage and limited mobility.

Risk Factors for Gout

Several factors can increase the risk of developing gout. These include:

  • Diet: A diet high in purine-rich foods such as red meat, shellfish, and sugary beverages can raise uric acid levels in the blood. Additionally, excessive alcohol consumption, especially beer, can increase the risk of gout.
  • Obesity: Being overweight increases the production of uric acid in the body, as well as reducing the kidneys’ ability to eliminate uric acid efficiently.
  • Medical Conditions: Certain health conditions, such as high blood pressure, diabetes, heart disease, and kidney disease, are associated with an increased risk of gout.
  • Family History: A family history of gout may increase your likelihood of developing the condition, as genetic factors can play a role in how the body processes uric acid.
  • Medications: Some medications, such as diuretics (water pills), low-dose aspirin, and immunosuppressants, can increase uric acid levels, making gout more likely.
  • Age and Gender: Gout is more common in men, primarily due to naturally higher levels of uric acid. Women’s risk increases after menopause, as estrogen levels decrease, which affects uric acid levels.

Diagnosis

If you are experiencing symptoms of gout, it is essential to get a proper diagnosis from a healthcare provider. At our Sunshine Coast clinics, we use a variety of methods to diagnose gout and differentiate it from other forms of arthritis:

  • Physical Examination: Our podiatrist will conduct a physical examination of the affected joint, checking for signs of inflammation, redness, and tenderness.
  • Joint Fluid Test: Completed by your General Practicioner, a joint fluid test, also known as synovial fluid analysis, involves extracting fluid from the affected joint using a needle. The fluid is examined under a microscope to detect the presence of urate crystals.
  • Blood Tests: Completed by your General Practitioner Blood tests can measure the levels of uric acid in the blood, although elevated levels do not necessarily confirm gout. Some individuals with high uric acid levels never develop gout, while others with normal levels can still experience gout flare-ups.
  • X-ray and Imaging: In chronic or advanced cases of gout, imaging tests such as X-rays, ultrasounds, or CT scans may be used to detect joint damage and the presence of urate crystals. We write you a referral to an imaging facility.

Treatment to Manage Gout:

In cases where gout affects the feet, our podiatrist can provide custom orthotics and footwear advice to alleviate pressure on the joints and improve mobility. We may also recommend exercises to strengthen the muscles around the affected joints, improving flexibility and reducing pain. Various medication prescribed by your General Practitioner can also help manage symptoms and pain.
