Iliotibial Band Syndrome (ITBS) is an overuse injury causing pain on the outside of the knee, commonly affecting runners and athletes. It occurs due to friction between the iliotibial band (ITB) and the femoral condyle during repetitive knee bending. Causes include tight ITB, weak hip and core muscles, flat feet, improper running technique, and unsupportive footwear.

What is Iliotibial Band Syndrome?

Iliotibial Band Syndrome (ITBS) is a condition that involves damage and inflammation of the iliotibial band (ITB), a thick band of connective tissue running along the outside of the thigh, from the hip to the shin. The ITB crosses the outside of the knee, where repetitive friction against the femoral condyle (a bony bump on the knee) can cause pain and irritation. ITBS is commonly experienced by runners and athletes involved in activities requiring repetitive knee bending.

Causes of ITBS

ITBS is primarily considered an overuse injury. It occurs due to repetitive movements of the ITB that involve the bending of the knee, such as those seen in running, cycling, and other forms of physical exercise. Several factors increase the likelihood of ITBS:

  1. Tight Iliotibial Band: A lack of flexibility in the ITB itself can cause excessive rubbing on the femoral condyle.
  2. Weak Hip and Core Muscles: Particularly weakness in the quadriceps and gluteus medius muscles can lead to improper knee movement and increased ITB tension.
  3. Flat Feet (Pronation): This can alter the biomechanics of the leg, increasing stress on the ITB.
  4. Abnormal Foot Biomechanics: Any deviation from normal foot mechanics can increase ITB strain.
  5. Limb Length Discrepancies: Unequal limb lengths can cause uneven stress and tension on one side of the body.
  6. Improper Running Technique: Running form that emphasises repetitive knee bending can increase ITB tension.
  7. Unsupportive Footwear: Shoes that do not provide proper support can exacerbate the condition.
  8. Training on Hills: Running uphill increases ITB strain due to increased knee bending.
  9. Restricted Fibular Glide: Limitations in the movement of the fibula during foot dorsiflexion can impact ITB mechanics

Symptoms of ITBS

The primary symptom of ITBS is pain on the outside of the knee, typically worsened by activities that involve knee bending. Other symptoms can include:

  • Tenderness or Burning Sensation: Often felt on the outside of the knee.
  • Swelling: Around the affected area.
  • Pain on Knee Bending: Especially noticeable at 30 to 45 degrees of flexion.

Diagnosis of ITBS

Diagnosis of ITBS typically involves a clinical examination and a review of the patient’s medical history and activity level. Our Podiatrists may also:

  • Running Technique: Ensure proper running form that minimises excessive knee bending and promotes an even gait.
  • Consistent Stretching: Regularly stretch the ITB, hip, and leg muscles to maintain flexibility.
  • Strengthening Exercises: Focus on strengthening the hip, gluteal, and core muscles to improve overall stability and biomechanics.
  • Supportive Footwear: Wear shoes that provide adequate support and cushioning to reduce ITB strain.
  • Custom Orthotics: If you have abnormal foot biomechanics, custom orthotics can help distribute stress more evenly and prevent ITB irritation.
  • Avoid Overuse: Monitor your activity levels and avoid repetitive activities that can lead to overuse injuries.
  • Surface Variation: Vary running surfaces to reduce continuous stress on the ITB.
