What is a corticosteroid injection?

A corticosteroid injection is a prescription anti-inflammatory medication placed into or around a painful structure, such as a joint, bursa, tendon sheath or other soft tissue, to help reduce inflammation. In foot and ankle podiatry, this may be considered for carefully selected conditions where inflammation is contributing significantly to pain and dysfunction.

Some injections may also include a local anaesthetic, which can provide short-term numbness or early pain relief.

At Elite Foot Care, we are able to perform ultrasound-guided corticosteroid injections in clinic.

Ultrasound guidance allows the treating podiatrist to visualise the target structure in real time and guide the injection more precisely to the intended area. In a foot and ankle setting, this can be particularly useful when treating smaller joints, tendon sheaths, bursae or deeper soft tissue structures where accuracy matters.

This also allows us to assess the area dynamically during the appointment and helps ensure the injection is delivered to the correct structure based on the clinical findings and ultrasound appearance.

How corticosteroid injections work

Corticosteroids work by reducing inflammatory activity within the targeted tissue. When inflammation is a major driver of pain, swelling or stiffness, an injection can help calm that response and create a window for improved function and rehabilitation.

These injections are not a cure for every condition and they are not a replacement for proper diagnosis, load management or rehab. They are one treatment option within a broader foot and ankle management plan.

When a foot and ankle injection may be considered

A corticosteroid injection may be considered when:

  • pain has not improved enough with appropriate conservative care
  • inflammation is believed to be a significant part of the problem
  • the painful structure has been properly assessed
  • reducing pain may help progress walking tolerance, footwear use or rehabilitation
  • the likely benefits outweigh the risks

In podiatry, this may apply to selected joint, bursal, tendon sheath or soft tissue conditions affecting the foot and ankle.

Our assessment process

Before considering an injection, we assess the foot and ankle properly. That matters, because a poorly targeted injection or an injection given for the wrong diagnosis is poor treatment.

Your appointment may include:

  • detailed history of your symptoms
  • clinical examination of the foot and ankle
  • review of previous treatment
  • review of relevant imaging where available
  • in-clinic ultrasound assessment where appropriate
  • discussion of whether ultrasound-guided injection therapy is suitable, or whether another treatment pathway is better

Not every patient is suitable for corticosteroid injection. In some cases, a different treatment approach is more appropriate.

What to expect after the injection

After a corticosteroid injection, it is common to have some temporary soreness at the injection site. Pain, swelling or bruising can occur for a few days. You may also be advised to temporarily reduce high-load activity after the injection depending on the structure treated.

The injection may take several days to begin working. For some patients, it can help reduce pain enough to allow better walking comfort, return to footwear, improved daily function or progress with rehabilitation.

Risks and side effects

This part should be stated clearly.

Possible side effects and risks can include:

  • temporary pain or flare-up after injection
  • bruising or swelling at the injection site
  • skin thinning or skin colour change in the treated area
  • infection
  • temporary rise in blood sugar, especially in people with diabetes
  • temporary rise in blood pressure
  • sleep disturbance or flushing
  • weakening of surrounding tissue depending on the structure treated

Repeated injections also need caution. Corticosteroid injections should be used selectively and for the right indication, not as a shortcut for poor diagnosis or incomplete management.

Is a corticosteroid injection the right choice?

Sometimes yes. Sometimes no.

A corticosteroid injection can be useful when the diagnosis is clear, the tissue target is appropriate, and inflammation is a major contributor to symptoms. But it is not a magic fix, and it is not appropriate for every foot or ankle problem.

In many cases, injection therapy should sit within a broader plan that may also include footwear advice, offloading, orthotics, strengthening, load management and rehabilitation.
